When an AI just makes things up — fake sources, fake quotes, entire fake court cases — and states them with total confidence. It isn't lying, exactly. It doesn't know the difference.
I asked it for citations. Every single one was hallucinated. Gorgeous formatting, though.

Rewriting working code to be 'cleaner.' Either essential maintenance or an elaborate way to avoid the actual ticket, depending on the week.
Spent the whole sprint refactoring instead of fixing the bug. The code is beautiful and still broken.

Retrieval-Augmented Generation. Giving an AI a search tool so it looks things up instead of Hallucinate-ing them. The duct tape holding most AI products together.
We bolted RAG on so it stops confidently making up our own documentation.
